Carla landed her dream job at the Ottauquechee Health Center in 2014 as the Community Care Coordinator. Carla embraces her robust team of community partners who support and inspire her work with patients and families. She has over 25 years of diverse experience in case management, including VT DCF-Division of Economic Services and Council on Aging S.E. VT.
Carla works to improve the quality of life and maintain independence for patients by coordinating multiple community partners in an interagency care management process. Carla provides patients with information about and referrals to community support services and assists with access to behavioral and medical care, from arranging transportation to locating sources of financial support. Carla’s approach is person-centered and meeting people where they are at in the community or their home. Her philosophy is to take the time to listen, truly listen, to someone in need, to best serve them.
Carla is a member of Mt. Ascutney Hospital and Health Center’s Community Health Team, facilitator of the Caregiver Support Group-Thompson Senior Center, a member of Pivotal Steps, and served on the boards of Scotland House Adult Day and Springfield Supported Housing Program.
